Thank you to everyone who shared their feedback on the Care in the Community poster. Your ideas helped inform the design and information to be included about the program.

The poster is now in health facilities across Western NSW Local Health District.

Would you like help getting care in the community? Call 1800931975. Our nurses and Aboriginal Health Workers can work with you to help you understand and manage your health conditions.

About the project

Planned Care for Better Health is a program delivered across all our communities to support people to stay well in the community. We aim to deliver care to people as close to home as possible and link you to health and social care services that you need.

Please share your ideas on the Care in the Community poster. The poster will be in hospitals and health facilities. Your input will help ensure the resource provides information to patients, families and carers to help them access support.
